How to discontinue a Care Plan a within a patient record.

From the Requests / Care Plans section:

1. Right click on the relevant plan in the ‘Plan’ section.
2. Select Discontinue.
3. Choose any plans of the plan you would like to keep, if any.
   E.g. you may no longer require the other interventions in the Care Plan but you may still want to record in the Food Chart
4. Click OK.
5. Complete Ordering clinician details if prompted.
6. Click Orders For Signature and then Sign.
7. The Care Plan will show as Discontinued in the Plans section.
