mediSIGHT – Recording a DNA

Background

When a patient does not arrive for their appointment (clinic, theatre or any other appointment) it is important to make a record in the notes.  This can assist in enquiries later, or highlight when a patient is in need of additional support. This may include WNB “Was Not Brought”, DNA “Did Not Attend”, UTA “Unable To Attend”.

It may be desirable to also generate a letter to the patient.  Currently this should be done in DocStore in Powerchart.

Note that this guide refers to documenting the DNA, and clinical decision regarding review.  The documentation on mediSIGHT does not have any effect on patient care.  It is critical that an 18 week wait tracker form is completed, and any correspondence is created in DocStore.
Process
  1. Open mediSIGHT and find the patient.
  2. Click “Create new +” and choose “DNA”

  1. Ensure the correct Episode and Pathway are chosen.
  2. Choose and appropriate Reason
  3. Click “Plan” and add a note stating the DNA and whether the patient is to be discharged or rebooked.
  4. If the patient is to be discharged, ensure that you click Discharge to close the pathway

  1. Click “Finalise” to save the record.
Writing to the patient / GP
  1. Letters to the patient / GP are not currently generated by mediSIGHT
  2. Letters should be written by the secretary, then checked and signed by the consultant, on DocStore
