mediSIGHT – Nurse Assessment in Virtual Clinic

Procedure for recording a clinical assessment in any virtual (or uncoupled) assessment clinic.  The patient will be present during this assessment, with the clinical details acquired sent to a clinician for review after the patient has left.

A report will be sent to the patient and GP when the data is reviewed, but will not be sent following the initial assessment clinic.

1. Open mediSIGHT and find the patient
2. Click “Create new +” and choose “Assessment”

3. Set the Visit Assessment Details
   1. Pathway
      It is essential to select the correct clinic type.  This cannot be changed later.
      Virtual assessments are only available in some pathways, e.g. Glaucoma, VR, MR and Neuro.
   2. Visit Date – date when the patient was in clinic, being assessed)
   3. Seen by – name of the staff member assessing the patient
   4. Location – Newcastle Eye Centre > Clinic
   5. Outpatient
   6. For review? “Yes”
      “Yes” flags the assessment for review at a later time by a clinician.  It should not be used for face to face clinics.
   7. Reviewer – choose the Reviewer Group
      Do not assign virtual patients to individual consultants. Ensure that a group is always selected.  If you are unsure which group then please speak to the clinical team.
   8. Responsible – Lead Consultant, if known

4. Complete the rest of the assessment as per the Assessment Guide.
