DNACPR Decision

The following detailed guide will show how to access the DNACPR forms within PowerChart and show all the new features.
Instruction
1. Open a Patient’s record within PowerChart.
2. Click on the patient’s name in the blue bar to open the Advanced Care Planning window – this is the one place to easily access the latest details and update them.

3. From the Treatment Escalation and Resus Status component,
click on the + button and select Treatment Escalation Plan
and Resuscitation Status to open the form.

4. In the Escalation Plan page, complete field details:
- What is the resuscitation plan? – DNACPR.
- Is this a copy of an existing valid community/paper DNACPR? – No.
- What is the escalation plan?
- Are there any other specific treatments which would or would not be appropriate for this patient?
- Additional Documentation Details section – there are conditional fields which will produce mandatory fields dependant on the response to certain questions.
- Is a Mental Capacity Assessment necessary?

5. Click on the DNACPR decision page, and complete details for sections:
- Reason for DNACPR decision
- Has there been…
- Court Involvement
- Transferring information
- DNACPR Discussion
N.B. No need to document the TEP discussion in the form however this section can be completed if
TEP discussion takes place at the same time.

6. Click on Signing page, dependant on the counter signature selected, you will have additional
mandatory fields to complete:
- Junior Doctor – Will need countersignature


- No Countersignature required

7. Click green tick top left-hand side to sign the form.

8. Click on the refresh button.
- Key details from the form are now visible in the workflow page

9. Close the Advance Care Planning form and refresh the patient record, the Resus status in
the banner bar has updated to be followed by today’s date.
- If countersignature is “Junior Doctor – will need countersignature”, the resus status will
show:

- If countersignature is “Countersign now”, the resus status will show:

- If countersignature is “No countersignature required”, the resus status will show:

If a countersignature is required, then senior clinicians will need to countersign the form, follow the Countersigning a DNACPR decision user guide.
If no countersignature is needed Senior Clinician’s will then need to access Document Creation to create a DNACPR form
