How to create a document

Scenario

You want to create a new document or letter for a patient.

Instructions
  1. With a patients record open, select Document Creation in the blue menu.
  2.  The ‘New Letter’ window opens.
  3. Ensure the Encounter is correct.
  4. Department auto-populates based on the encounter chosen.
  5. Select the correct Letter type.
  6. Set the Primary Recipient.
  7. Click Create. The document creation will open.

  1. Within the document, the Pencil icon indicates free text area.
  2. To add autotext, select the Autotext tab found on right side of page. Personal autotext can be created, or departmental or Trustwide auto text can be used.
  3. To add data from the patient’s record select the Data tab. Click an item to insert that piece of data into the document.
  4. Check that the correct details are displayed in the Details tab located on the right side of the page.
    • Department and Lead Clinician will auto-populate based on the encounter chosen in ‘New Letter’ window.
    • Clinic auto-populates based on what was chosen in the ‘New Letter’ window. Change as required using drop-down list.
    • Select your name from the Signing Clinician list.  The green button at bottom of page should now read as Sign and Send.
    • Select your role from the drop-down list.

  1. At the bottom right the following options will display. Select the appropriate option:
    • Cancel – cancel the creation of the document without saving it.
    • Save as a Draft – will save a draft into the Draft Documents List.
    • Save and Finalise – this option will display if you are not signed into the PC or you have not selected yourself as the signing clinician. The document will need to be signed via the Document Signing list by the Lead or Signing Clinician.
    • Sign and Send – this option will display if you are correctly logged in and you are selected as the Signing Clinician.

Once signed the document needs to be sent via the Document Store Print App. This is usually done by a sectary or admin support.

If you have written your own letter, to ensure that it is sent,  you must let your clinical administrator know.  The signed document will sit in a print queue for clinical administrator to action sending.
