How to create a Diagram

Enter PowerChart and from the blue menu on the left of the screen  -select the “Diagram Creation” Option.

You should get a message in the screen saying “Diagram Creation has opened in your internet explorer browser” and “Please refresh to open again” along with the date and time you attempted to open the menu.  If you select the refresh button in PowerChart, it will open the Diagram Creation window in an additional browser tab.

The window (shown below) should open automatically.  Your patient’s MRN, NHS number, Name and Date of Birth should be shown across the top of the screen.

1. Select your department from the drop down menu in the top left.
2. Select the background you would like for your diagram.
3. Edit the background as needed
4. Save Image

How to Edit a Diagram

You can use the tools on the left hand side of the screen to annotate the background.  To do this, select the tool you want (dot, straight line, square or circle) and click on the area of the diagram you wish to annotate.

• For lines, squares and circles, you will need to drag the mouse to set the size and direction of the shape.
• The dot tool can be used to draw a curved line.
• Widths will change the thickness of the tool selected, and you can also change the colour using the colours option further down the menu.
• You can also insert text.  To do this, click on the “A” on the left hand menu and then click on the diagram where you wish to place the text.  You should then be able to fill out the text as needed.
• If you add anything into the diagram accidentally, you can use the “undo” button in the top right corner to undo your most recent action.  Alternatively the reset button will remove all annotations from the diagram.
• If you wish to delete a specific annotation, you can select annotations using the move tool and either move them or delete them by pressing the cross shown when they are highlighted.

How to save a Diagram

Once you have completed your diagram – press “Save Image” in the top right corner.  This will open a window asking you to confirm the department and to provide a description of the diagram.  Once you have done this, press “Save” or press “Cancel” to continue editing.  Saving will then clear the screen ready for further diagrams.
