How do I access Ward Dashboard?
In Powerchart.
- Click View then Ward Dashboard - you should also be able to see and click on Ward Dashboard in the row below.
How do I access the Discharge Dashboard?
- Click on the Discharge tab.
What is it?
- The Discharge Dashboard is a list that summarises the discharge plans for a group of patients.
- It is the place in eRecord to see and update information about a patient’s discharge.
- The list of patients can be sorted by the expected date of discharge to see who is almost ready to be discharged on a ward.
- You can complete the new Discharge Hub Referral form from this page.
How do I set it up?
- Click Select List.
- Click Patient list.
- Click List Maintenance.
- In the opening Patient list window, any lists you have already set up for Care Compass will appear in the Available lists column.
- Select the list you want to set up, then click the right arrow in the middle to pull it across into the Active lists column.
- Click OK.
- The list should now be appearing and a list of Patient’s from that group will be showing.
How do I establish relationships?
- Your Patient List will now be showing.
- Some patients may have inactive fields (boxes will appear grey).
- Click Establish Relationships.
- Click ‘select all’ and then ‘submit’.
- The page will reload, this may take a few moments.
What does the dashboard show me and what can I do with it?
The dashboard will give you a ward overview of the discharge status of the patients on your ward. These include:
- Patient - patients name and MRN
- Location - Ward number and bedspace
- Visit - Admission date and length of stay
- Isolation - Isolation status
- Actions - This can be used as a free text box to update outstanding MDT discharge actions. E.g., ‘Equipment being delivered 25th September’, ‘Stair assessment needed’, ‘District nurse referral required’. Information entered here will not pull into the patients record, clinical notes should be recorded as usual.
- Pathway - Discharge pathway
- Ready for discharge - Date the person no longer meets the criteria to reside.
- Documentation - Indicates if a board round form or discharge hub referral form has been completed.
- Expected date of discharge - Current expected date that the patient will be able to leave hospital. Use this field to update the board round form.
- Resus - Resuscitation status.
- Discharge medications - Progress of discharge medications within pharmacy.
How do I customise the columns and rows?
- Click on the drop-down arrow in the top right-hand corner of the screen and select Layout Configuration.
- Select changes to customise the columns.
- Select Rows.
- Select changes to customise the rows.
