How to discharge a Postnatal Inpatient to Community

1. Open mother’s record in BadgerNet.
2. Click on the Postnatal tab.
3. From the Add new note dropdown, select Transfer of Care Smart Form (Woman).
4. In the Transfer of Care Smart Form, select Postnatal Community Care for the Transfer Type field.
5. Complete details then select Save and Close
6. Under Reports, select Clinical Reports.
7. Select Admissions and Discharge, then Mother Transfer to Postnatal Community Care.
8. Click Confirm and Save in the top right corner of the form.
9. Under ‘Baby 1’ tab, click on ‘Summary of Care’.
10. From the Add new note dropdown, select Transfer of Care Smart Form (Baby).
11. Complete details, then select Save and Close.
12. Under ‘Baby 1’ tab, click on ‘Reports’.
13. Select Admissions/Transfer, then Baby Transfer to Postnatal Community Care.
14. Click Confirm and Save in the top right corner of the form.
