1. In any suitable workflow tab, select the Diagnosis & Problems component
2. Use the drop down menu next to the search box to select This Visit, This Visit and Chronic or Chronic as appropriate
3. In the search box, type the name of the diagnosis/ problem and select from the list
4. The diagnosis or problem will be added to the record
5. You can modify whether this is a This Visit or Chronic problem using the buttons on the right
6. Select the diagnosis or problem to see more detail in the pop up on the right
7. Comments can be added using the comments box at the bottom of the details pop-up
8. Multiple This Visit diagnoses can be prioritised using the drop down to the left of each diagnosis

If the diagnosis or problem does not appear on the list you can add a free text diagnosis/ problem. If you accidentally untick both the This Visit and Chronic buttons next to the diagnosis/ problem it will disappear. You can add it again or re-activate it
