TEP Guide – For CPR

User guide for when a  clinician is required to complete a TEP form for the patient with a resuscitation status of “For CPR”.

Instruction

  1. Open the Patient record.
  2. Click on Treatment Escalation Plan in the blue menu to open the Advanced Care Planning window – this is the one place to easily access the latest details and update them.

  1. From the Treatment Escalation and Resus Status component, click on the + button and select Treatment Escalation Plan and Resuscitation Status to open the form.

  1. In the escalation plan page, complete all fields:
  • What is the resuscitation plan? – select “For CPR”.
  • What is the escalation plan? – select appropriate plan.
  • Are there any other specific treatments which would or would not be appropriate
    for this patient? – these details will pull through and be visible in the workflow page.
  • Additional Documentation Details.
  • Is a Mental Capacity Assessment necessary? – select the appropriate choice

Note – You do not need to complete the DNACPR decision page.

  1. In the TEP Discussion page, add Treatment Limitation details and document the TEP Discussion

  1. In Signing, complete signing name, status and GMC/MNC number details.

  1. Click green tick in the top left-hand side of the window to sign the form.

  1. Close and re-open the Advanced Care Planning form to refresh.
  •  Key details from the form are now visible in the workflow page.
  •  Treatment limitations are clearly displayed without needing to open the previously
    completed form.

  1. The TEP level selected is now visible in the banner bar (you may need to refresh the patient’s chart
    to view this)
