Trust policy requires all patients 16 years or older to have a VTE Risk Assessment completed on admission and when clinical condition changes during the admission.

To complete the assessment via AdHoc forms follow these steps:

  1. Click on the AdHoc button on the menu bar at the top of the screen  AdHoc Button
  2. Select VTE Assessment NUTH
  3. Click the Record button
  4. Complete the assessment and sign

If the VTE risk assessment has not been completed within 24 hours an alert will be displayed when the patient record is opened.

  1. User opens patient record
  2. Completion of VTE Risk Assessment required warning box displays
    • Click the VTE Risk Assessment button to complete the form
    • To cancel opening the record select Cancel and close record and click OK
    • To proceed without completing the form select Emergency Override and click OK

The VTE Risk Assessment must be documented in the record before anticoagulant medication can be prescribed. If it has not been documented the user will be prompted to do so before continuing.

  1. User attempts to prescribe anticoagulant medication
  2. Completion of VTE Risk Assessment required warning box displays
    • Click the VTE Risk Assessment button to complete the form
    • To cancel prescribing the medication tick the CANCEL ORDER box and click OK

If the warning has been cancelled or overridden it will not display again for 2 hours. The assessment can be completed at any time via AdHoc forms.
