Trust policy requires all patients 16 years or older to have a VTE Risk Assessment completed on admission and when clinical condition changes during the admission.
Complete Via AdHoc
To complete the assessment via AdHoc forms follow these steps:
- Click on the AdHoc button on the menu bar at the top of the screen.
- Locate VTE Assessment NUTH, complete the form and sign.
Complete via 24 hour alert
If the assessment has not been completed have 24 hours an alert will display. You have the options to:
- Complete the assessment via the VTE Risk Assessment button.
- Cancel and close the patient’s record.
- Emergency Override to proceed to open the patient’s record.
Complete via prescribing alert
If the assessment has not been completed an alert will display when the user attempts to prescribe certain medications. You have the options to:
- Complete the assessment via the VTE Risk Assessment button.
- Cancel prescribing by ticking the CANCEL ORDER box and clicking OK.
If the warning has been cancelled or overridden it will not display again for 2 hours. The assessment can be completed at any time via AdHoc forms.
