This process will show how to create a Nursing Handover form for a patient together with a report of all handovers for patients on a ward.

Creating the Adhoc Form

• Open a patient record in PowerChart
• Select the Adhoc button from the toolbar

• Select ‘Nursing Handover‘ adhoc form by ticking the box to the left of the form
• Click the Record button to open the form

• Once the form is completed, sign the form clicking on the green tick in top left hand corner of the form

• Close the forms window
• The Nursing Handover completed form can be viewed in a number of places from the left side menu:
  • Document Viewing
  • Adhoc Forms (Double click on a Form to open in a Read Only format)
  • Results Review (the Documents tab)

Modify an Adhoc Form

• In the left side menu select Adhoc Forms
• Right click the form and select Modify from the menu
• Make the necessary amendments and sign the form again using the green tick

Creating the Ward Handover Report

• From the eRecord Apps page
• Open the Reporting Portal app

• Locate the report Handover Ward (tip: you can click the star to add this to your My Favourite reports for next time)
• Click the Run button

• Do not change the Output to File printer option (once the report is created it can be printed if required)
• In the Facility field select the hospital site for your ward
• Select the Building site from the Building field
• Locate and select your ward
• Click the Execute button
• Once the report has been created, it can be printed using the printer icon located top left of the report
