How to discharge a patient from Community Care to Health Visitor

1. Open mother’s record in BadgerNet
2. Click on the Postnatal 
3. From the Add new note dropdown, select Transfer of Care Smart Form (Woman).
4. In the Transfer of Care Smart Form, select Community to Health Visitor/FNP for the Transfer Type
5. Complete details then select Save and Close
6. Under Reports, select Clinical Reports.
7. Select Admissions and Discharge, then Transfer of Care from Community to Health Visitor
8. Click Confirm and Save in the top right corner of the form.
9. Under ‘Baby 1’ tab, click on ‘Summary of Care’.
10. From the Add new note dropdown, select Transfer of Care Smart Form (Baby).
11. Complete details, then select Save and Close.
12. Under ‘Baby 1’ tab, click on ‘Reports’.
13. Select Admissions/Transfer, then Baby Transfer of Care from Community to Health Visitor.
14. Click Confirm and Save in the top right corner of the form.
