Accessing the Antenatal Booking Form

  1. In the pregnancy record, select Notes During Pregnancy from the left menu
  2. Click the red plus button to the top left
  3. From the drop-down menu select Antenatal Booking

Overview of the Antenatal Booking Form

The Antenatal Booking form is a smart form that allows you to complete several separate forms at the same time. The sections to the left of the form can be worked through one after the other – there is no need to save and close the form after completing each section!

  1. On Arrival – start date and time of the booking, location and type of assessment. Also allows import of information from the Portal Pre-Booking form
  2. About the Woman – access to the Portal Access and Demographics forms
  3. Current Pregnancy Details – dates of LMP, Agreed EDD, Referral and Menstrual, Fertility and Contraception information
  4. Obstetric History – allows import of previously recorded Obstetric History if this is available on BadgerNet
  5. Assessment – record Observations
  6. Health History – Medical History including Allergies, Mental Health, Family Health, Smoking and Alcohol
  7. Social and Confidential – Safeguarding information including FGM.
  8. Screening – Booking Bloods & Microbiology screens, Vaccinations and Scans
  9. VTE Risk Assessment – Record the VTE risk
  10. Recommendations – System generated Risk Assessment & Management Plan that may be modified by the user
  11. Fetal Growth and Pre-eclampsia Risk Assessment
  12. Care Plan – Named Midwife and other professionals involved in care, intended location of birth
  13. Discussion – Record information discussed with the woman
  14. On Leaving – Assessment end date and time. If the referral needed checkbox is set to yes a Referral Form will open automatically when the Antenatal Booking form is Saved and Closed
