How to complete the self-referral form
This guide shows how to complete the self-referral form for the occupational health service.
To access the self-referral portal please visit the Occupational Health page on the intranet and select Self-referral information in the menu.

- A message will display. Please read the message as it contains important information related to your referral. When you are ready click Continue.
- Complete your personal details including name, date of birth and telephone.
- Select the reason for your request in the drop-down menu. The options are counselling, general or physiotherapy.
- The referral information section for the selected type of referral will display.
- Read the information provided and complete the details.
- Each referral form has questions specific to the type of referral being requested. Mandatory questions are marked with a red star.
- At the bottom you can attach any related documents. Drag and drop the file or use the Browser for files button. Once attached the file will display below. It is not mandatory to attach documents.
- Click Submit Form.
- A confirmation message will display. The occupational health team will be in touch once the request has been reviewed.
