Record a Patient with Accessible Information Needs<\/h1>\n

What is accessible information?<\/strong><\/p>\n

\u201cThe aim of the Accessible Information Standard is to make sure that people who have a disability, impairment or sensory loss get information that they can access and understand, and any communication support that they need.\u201d<\/p>\n

 <\/p>\n

What are the requirements?<\/strong><\/p>\n

Ask<\/strong> what communication or information needs a person has.
\nRecord<\/strong> those needs.
\nClearly Flag<\/strong> them so that other professionals will see and follow them.
\nShare<\/strong> the information with other professionals when consent has been given.
\nAct<\/strong> so that people can have information in the format that suits their needs and appropriate arrangements can be made for appointments.

How to Record a Patient’s Accessible Information Needs<\/div><\/p>\n

The following guide indicates where we record Accessible Information. Please note if you are working in an Inpatient setting or Outpatient setting you must record it in Powerchart.<\/p>\n

    \n
  1. From the Desktop of your computer, select the green eRecord<\/strong> icon.
    \n\"\"<\/li>\n
  2. From within eRecord, open the Powerchart <\/strong>application.
    \n\"\"<\/li>\n
  3. Find your patient using the MRN search box<\/strong> or search by name.
    \n\"\"<\/li>\n
  4. Select Diagnosis & Problems<\/strong> from the menu.<\/li>\n
  5. In the bottom part of the window (the Problems section) select +Add<\/strong>.
    \n\"\"<\/li>\n
  6. Under Classification<\/strong>, please ensure you select Accessibility<\/strong>.<\/li>\n
  7. Select the Folders<\/strong> option that is relevant to your patient\u2019s requirements and scroll to find and open the correct Accessible Information folder \u2013 there are 4 to choose from.
    \n\"\"<\/li>\n
  8. Find the term that is their actual need and double click to populate the problem field.
    \n\"\"<\/li>\n
  9. Click OK<\/strong>.<\/li>\n
  10. Once saved, the demographic bar will display **Flags**<\/strong>. This is a link, that will display the accessible information requirement(s) for the patient when clicked on.

    <\/i> When a patient is admitted, a window will display to identify the patient has Accessibility Information documented.<\/div>\n

    Outpatient Appointments<\/div>\n

    Once a patient has been recorded in PowerChart with Accessible information needs, the following guidance will show you how it appears in PM Office and SchApptBook.

    Outpatient Appointments – PM Office<\/div>\n

    Adding or viewing a referral.<\/p>\n

      \n
    1. Select PM Office<\/strong>.
      \n\"\"<\/li>\n
    2. Search for the Patient using any demographic Conversation<\/strong>. Note: In this example we will be using Outpatient Referral<\/strong>.<\/li>\n
    3. A \u201cpop up\u201d will display if a patient does have any accessibilty information. Click OK<\/strong>.
      \n\"\"<\/li>\n
    4. The Outpatient Referral displays Accessibility Information details at the bottom of the referral (you may need to scroll to view this).
      \n\"\"<\/li>\n<\/ol>\n

        Book \/ Confirm \/ Check In \/ Check Out \/ Cancel \/ DNA and Reschedule<\/strong><\/p>\n
