{"id":9469,"date":"2024-07-18T09:57:16","date_gmt":"2024-07-18T09:57:16","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=9469"},"modified":"2025-06-06T18:40:29","modified_gmt":"2025-06-06T18:40:29","slug":"fluency-direct-v12-guidance-for-clinicians-2","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=9469","title":{"rendered":"Document Creation Using Digital Dictation"},"content":{"rendered":"<ol start=\"1\">\n<li>You have launched Fluency Direct by double clicking on the desktop icon.<br \/>\n<strong>Note: Fluency Direct does NOT autostart.<\/strong><\/li>\n<p><\/p>\n<li>From within Powerchart, you have opened the patient\u2019s record on the correct encounter.<\/li>\n<p><\/p>\n<li>The patient\u2019s MRN will populate in the Pill. Confirm that the MRN in the Pill is that of the patient for whom you wish to dictate a document.<\/li>\n<p><\/p>\n<li>Select <strong>Document Creation<\/strong> from the blue, left-hand side menu.<\/li>\n<p><\/p>\n<li>Complete the new document details box.<br \/>\n<strong>Ensure you are documenting on the correct encounter.<\/strong><\/li>\n<p><\/p>\n<li>The selected template opens.<\/li>\n<p><\/p>\n<li><strong>Dictate directly into a text field within the template<\/strong>.<\/li>\n<p><\/p>\n<ol start=\"1\">\n<li>Click on a text field, indicated by a pencil icon on the right-hand side.<\/li>\n<li>Press and hold <strong>Record<\/strong> to dictate directly.<\/li>\n<li>Release <strong>Record<\/strong> when not dictating.<\/li>\n<li>Repeat steps 2 and 3 when you with to dictate.<\/li>\n<\/ol>\n<p>\n<div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  For reviewing the patient\u2019s record while dictating, you can use the Tear Off function. This will allow you to view the letter in one window and the patient record in another. <\/p>\n<ol start=\"1\">\n<li>Current window should be the clinical letter.<\/li>\n<li>Click on Tear Off in the top grey menu bar.   <img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/training.nuth-itt.co.uk\/wp-content\/uploads\/2024\/07\/Tear-off.jpg\" alt=\"\" width=\"81\" height=\"25\" class=\"alignnone size-full wp-image-9523\" \/><\/li>\n<li>The clinical letter window is now a new window and can be dragged to one side of the screen or onto another display screen.<\/li>\n<li>The background window is the patient record, and you can select whichever section you wish to review.<\/li>\n<p><\/div>\n<\/ol>\n<p><\/p>\n<ol start=\"8\">\n<li>There are two ways to correct mistakes.<\/li>\n<p>\n<strong>In-line Correction<\/strong> will only make a correction to the text in the current documentation,<\/p>\n<ol start'\"1\">\n<li>Say \u201c<strong>select [text]<\/strong>\u201d or use the keyboard and mouse to highlight text.<\/li>\n<li>Say \u201c<strong>[text]<\/strong>\u201d and the text is replaced.<\/li>\n<\/ol>\n<p> <br \/>\n<strong>Error Correction Window<\/strong> will force Fluency Direct to learn the word\/phrase.<\/p>\n<ol start=\"1\">\n<li>See the error in your dictated text.<\/li>\n<li>Say \u201c<strong>select [text]<\/strong>\u201d.<\/li>\n<li>Say \u201c<strong>correct selection<\/strong>\u201d to open the error correction window.<\/li>\n<li>You will be presented with a list of choices. To accept a choice, say \u201c<strong>choose [number]<\/strong>\u201d.<\/li>\n<li>Below the choices are a list of formatting commands. To use, say the command name.<\/li>\n<\/ol>\n<p><\/p>\n<li>Spell Mode will allow you to correct the spelling of a word.  It is limited to spelling only and does not present an option to save it to one\u2019s dictionary.<br \/>\n<\/p>\n<ol start=\"1\">\n<li>Highlight the word to correct.<\/li>\n<li>Enable Spell Mode by saying \u201c<strong>spell mode<\/strong>\u201d. <\/li>\n<li>Vocally spell the word.<\/li>\n<li>Say \u201c<strong>save\u201d<\/strong>.<\/li>\n<\/ol>\n<p><\/p>\n<li>The Fluency Direct Dictionary allows you to create a custom dictionary of Names, Locations, Medications, Procedures, Diagnoses, and Excluded Words that you use often to ensure recognition each time. <\/li>\n<p><strong>Adding a Dictionary Entry<\/strong><\/p>\n<ol start=\"1\">\n<li>Say \u201c<strong>add to dictionary<\/strong>\u201d.<\/li>\n<li>The Add a word window will appear.<\/li>\n<li>Enter the written for of the word to be added to the dictionary.<\/li>\n<li>Based on the written form, the pronunciation will be created. You can change this by clicking on the blue Change link.<\/li>\n<li>Select the <strong>Record pronunciation<\/strong> tab.<\/li>\n<li>Hold down the <strong>Record <\/strong>button and dictate the word.<\/li>\n<li>You will then be returned to the Add a Word dialog.<\/li>\n<li>Select the <strong>category<\/strong> for the dictionary entry.<\/li>\n<li>Click <strong>Save<\/strong> or say \u201c<strong>save<\/strong>\u201d to finish.<\/li>\n<\/ol>\n<p><\/p>\n<li>Fluency Direct allows for <strong>custom commands <\/strong>to be created that will automate single or multi-step actions with voice. These can be as simple as a voice command to pull in a snippet of text, or a complex set of actions that automate otherwise multiple-click workflows.\n<p><strong>How to create signature autotext<\/strong>:<\/p>\n<ol start=\"1\">\n<li>Press <strong>Play<\/strong> and say \u201c<strong>open commands<\/strong>\u201d.<\/li>\n<li>Select command group.  You should have a personal group at the top of the list called \u201cYour firstname Your surname\u2019s Commands\u201d.<\/li>\n<li>Commands within that group will be displayed to the right side of the window.<\/li>\n<li>Click <strong>+Add<\/strong> in the top right corner to create a new command.<\/li>\n<li>The Add a command window opens.<\/li>\n<li>Give the command a name.<br \/>\nTip: Keep the command name simple to pronounce. Capitalize the first letter of each word in the command name. Spell out numbers rather than using the numerals. Use an \u201caction verb\u201d at the beginning of each command name. Make sure the command is more than one word.<\/li>\n<li>In the Steps tab, choose \u201c<strong>Text<\/strong>\u201d.<\/li>\n<li>Type the signature text into the text field. <\/li>\n<li>The Details tab is useful if you wish to write a description for this command. You can also set command availability to be everywhere, only in certain applications, etc. \n<li>Click <strong>Save<\/strong>.<\/li>\n<li>Click <strong>Apply<\/strong> and then click <strong>OK<\/strong>.<\/li>\n<li>To use your new Autotext, place the cursor where you wish to insert.  Press <strong>Play<\/strong> and say \u201c<strong>insert signature<\/strong>\u201d.<\/li>\n<\/ol>\n<p>\n<!-- [infoSection]The custom command functionality is powerful. Be sure to click on each of the Actions subheadings in the left-hand side menu of the page that opens.[\/infoSection]\n<br \/> --><\/p>\n<li>When finished, you have several options:<\/li>\n<p><\/p>\n<ol start=\"a\">\n<li>Select <strong>Save as Draft<\/strong> if your document requires review by a clerical administrator.<\/li>\n<li>Select <strong>Save and Finalise<\/strong> if no review is required and you are not a signing clinician.<\/li>\n<li>Select <strong>Sign and Send<\/strong> if no review is required and you are a signing clinician.<\/li>\n<\/ol>\n<p>\n<div class=\"warnSection\"><i class=\"fas fa-exclamation-circle\" style=\font-size:3rem\"><\/i>  You must stop the recording BEFORE the document is saved as a draft. If you do not, NO AUDIO WILL BE RECORDED.<\/p>\n<p>To stop recording, either release the Record button on the SpeechMike, or click the microphone icon on the Pill so that it is no longer green.<\/div><br \/>\n<br \/>\n<div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  Once the document has been saved as a draft, the audio file is stored against it and cannot be amended. <\/p>\n<p>For example, a document is saved as a draft after the first round of dictating. If the author returns later and edits the document, NO NEW AUDIO WILL BE RECORDED against it. The audio file will not expand to include the new dictation. Only the voice recognition typing will work.<br \/>\n<br \/>\nHere is the workaround:<\/p>\n<ol>\n<li><strong>Duplicate <\/strong>the letter that you wish to edit.<\/li>\n<li>Dictate as per usual.<\/li>\n<li>Save the letter and the newly dictated audio file is available. Please note, the audio file for this letter only contains new content. It does not append new content to old content.<\/li>\n<li>Don&#8217;t forget to <strong>reject<\/strong> the original letter, when appropriate, so that only one copy, the edited copy, remains.<\/li>\n<\/ol>\n<p><\/div>\n<\/ol>\n<p><\/p>\n<ol start=\"13\">\n<li>If your document requires review by a clerical administrator, you may choose to communicate with them verbally, via email or Message Pad.<\/li>\n<p>\nTo use Message Pad:<\/p>\n<ol start=\"i\">\n<li>Select <strong>Message Pad<\/strong> in the blue menu bar at the top of the document.<\/li>\n<li>Click on <strong>New Message<\/strong>.<\/li>\n<li>Select message <strong>Priority<\/strong>.<\/li>\n<li>Type message.<\/li>\n<li>Click <strong>Add<\/strong> to save.<\/li>\n<li>The clerical administrator will see the message in Typing Shelf and action accordingly.<\/li>\n<\/ol>\n<p><\/p>\n<li>Dictate one letter for one patient at a time. Save as Draft before moving on to the next patient.<\/li>\n<p><\/p>\n<li>Always check the MRN in the Pill matches that of the record you are going to dictate against.<\/li>\n<p>\n<div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  If you find that an MRN is stuck in the pill \/ not dictating against a patient record, press Play and say \u201cClear MRN\u201d. This removes the association of the dictation to a patient. Reassociation can be achieved by opening the patient record again or looking at a page within it.<\/div><br \/>\n<\/p>\n<li>Remember, if the pill is green, audio is recording, and voice recognition typing is working. If the pill is blue, audio is recording but there will be no voice recognition typing.<\/li>\n<p><\/p>\n<li>For best results, train your profile by correcting words and build up your dictionary.<\/li>\n<p><\/p>\n<li>If you find that your voice recognition is poor, particularly if you have not had the time to train your profile in the past, as a last resort, you can request via Service Desk to have your profile deleted\/reset.
