Recording a Referral In (Triage, Assessment, Declined and End)<\/h1>\n
Information<\/div>\n

Referrals are integral part to working with patients.\u00a0 They record your clinical and non-clinical activity into funding.\u00a0 Recording a referral can be scary, hence why this guide has been created.<\/p>\n

All referrals must be recorded, and clinically triaged before being “accepted or rejected”.<\/p>\n

Record a Referral in<\/div>\n

  • Register the patient and record the referral in completing the relevant fields – the referral should have the following selected;<\/li>\n<\/ul>\n

    Service Offered \u2013 Choose relevant option<\/strong><\/p>\n

    Urgency \u2013 Routine or Urgent \u2013 This may change after triage.<\/strong><\/p>\n

    Referral Source \u2013 Choose applicable.<\/strong><\/p>\n

    Referral in Reason (primary) \u2013 Choose applicable.<\/strong><\/p>\n

    Outcome \u2013 Choose applicable (see below)<\/strong><\/p>\n

    Referral Status \u2013 Choose applicable (see below)<\/strong><\/p>\n\n\n\n\n\n\n\n

    After the Initial Appointment with patient<\/strong>, the status <\/strong>of the referral must be set to \u2018Receiving Care\u2019. <\/strong>This is to ensure waiting times can be measured.<\/p>\n

    IMPORTANT<\/strong> when saving the initial appointment, ensure the saving event is Linked to referral<\/strong> – Choose the relevant referral<\/strong><\/p>\n

    Click Update Status, click close,<\/strong><\/p>\n

    Referral Declined – Add Referral<\/div>\n

    Register the patient and record the referral in completing the relevant fields – the referral should have the following selected;<\/p>\n

    Referral Status \u2013 Change to Rejected \u2013 Inappropriate Referral \u2013 select OK<\/strong><\/p>\n

    The \u2018End Referral\u2019 pop up box will appear, select the following options;<\/p>\n

    Reason for End of Referral \u2013 Discharge<\/strong><\/p>\n

    Date of Discharge \u2013 Enter relevant date.<\/strong><\/p>\n

    Location after Discharge – Choose applicable.<\/strong><\/p>\n

    Interventions \u2013 Inappropriate Referral <\/strong><\/p>\n

    Select OK<\/strong><\/p>\n

    Save the record > Select the \u2018Event<\/strong> Template<\/strong> \u2018Administration>\u2019<\/strong> and \u2018Apply Template\u2019.<\/strong><\/p>\n

    IMPORTANT<\/strong> \u2013 Tick include \u2018Ended Referrals\u2019<\/strong><\/p>\n

    Link to referral<\/strong> \u2013 from the drop down choose the relevant referral<\/strong> which was created and ended.<\/p>\n
