{"id":6157,"date":"2023-11-10T19:21:36","date_gmt":"2023-11-10T19:21:36","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=6157"},"modified":"2026-02-13T10:41:43","modified_gmt":"2026-02-13T10:41:43","slug":"discharge-medications","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=6157","title":{"rendered":"Discharge (TTO) Medications"},"content":{"rendered":"<p>From within the discharge summary screen (to <a href=\"https:\/\/training.nuth-itt.co.uk\/erecord\/powerchart\/discharge-summary\/how-to-access-discharge-summary-screen\/\">access<\/a>), this guide describes how to:<\/p>\n<ul>\n<li><strong>convert<\/strong> medications to prescription, if required<\/li>\n<li><strong>edit<\/strong> to add new discharge prescriptions<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span data-teams=\"true\"><div class=\"guideSection\" id=\"\"><\/span>Convert medications to prescription<span data-teams=\"true\"><\/div><\/span><\/p>\n<ol>\n<li>Click <strong>Search<\/strong> and select your <strong>ward<\/strong> from the list.<\/li>\n<li>In line of correct patient, click <strong>Convert<\/strong> in Meds column. This opens patient record to the Medication List section.<\/li>\n<li>Ensure the display filter at the top of the medications listed is appropriate.\u00a0 More filter preferences are available if click on &#8230; symbol.<br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-6191\" src=\"https:\/\/training.nuth-itt.co.uk\/wp-content\/uploads\/2023\/11\/display.jpg\" alt=\"\" width=\"286\" height=\"36\" \/><\/li>\n<li>Select medication\/s as appropriate. Right click and select <strong>Convert to Prescription<\/strong>.<\/li>\n<li>In the details section at the bottom, complete the <strong>Duration<\/strong>, <strong>Pre-Visit Med<\/strong> and <strong>GP to continue<\/strong> fields. These are mandatory as they are in yellow.<\/li>\n<li>Click <strong>Orders for Signature<\/strong> in the lower right hand corner of the screen.<\/li>\n<li>Click <strong>Sign<\/strong> then refresh screen to review order\/s.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<div class=\"guideSection\" id=\"\">Add new discharge prescriptions<\/div>\n<ol>\n<li>Click <strong>Search<\/strong> and select your <strong>ward<\/strong> from the list.<\/li>\n<li>In line of correct patient, click on <strong>Convert<\/strong> in Meds column.
