{"id":5976,"date":"2023-11-06T15:07:14","date_gmt":"2023-11-06T15:07:14","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=5976"},"modified":"2024-05-03T10:22:01","modified_gmt":"2024-05-03T10:22:01","slug":"discharging-a-patient","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=5976","title":{"rendered":"Discharging a Patient"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p><strong>IMPORTANT: Discharging a patient to Transitional Care is a separate process which can be found<a href=\"https:\/\/training.nuth-itt.co.uk\/it-systems\/badgernet\/transferring-a-patient-to-transitional-care\/\"> <span style=\"text-decoration: underline;\">HERE<\/span><\/a><\/strong><\/p>\n<p>The standard discharge to home\/foster care process is:<\/p>\n<ol>\n<li>Click the &#8216;<strong>Discharges<\/strong>&#8216; title from the menu on the left hand side<\/li>\n<li>Click the &#8216;<strong>discharge details<\/strong>&#8216; button from the right hand side of this screen<\/li>\n<li>Complete the form (remember: it is possible to start this form, save and come back later to complete at any time)<\/li>\n<li>Save &amp; Close<\/li>\n<\/ol>\n<p><strong>Note<\/strong>: Saving &amp; closing this form will mean the patient is discharged at the time specified so it is best practice to fill out the date &amp; time after the patient has left to be most accurate.
