{"id":5335,"date":"2023-10-04T15:05:00","date_gmt":"2023-10-04T15:05:00","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=5335"},"modified":"2026-02-09T14:39:21","modified_gmt":"2026-02-09T14:39:21","slug":"review-hub-referral-form","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=5335","title":{"rendered":"Review Hub Referral form"},"content":{"rendered":"<p>To review the referral, in Task List:<\/p>\n<ul>\n<li>Double click on a task in the referral to open the Review Discharge Hub Referral form.<\/li>\n<li>Details from the referral form can be viewed in the Review form.<\/li>\n<li>Anything that needs to be corrected can be updated from the Review form.<\/li>\n<li>The Review section includes some mandatory fields to confirm which team is responsible for the discharge of the patient.<\/li>\n<li>Hub Comments should be used to reflect the next steps agreed and discussed in the Hub meetings.<\/li>\n<li>A follow-up task can be generated by selecting a date and time in the future.
