{"id":5325,"date":"2023-10-04T12:10:13","date_gmt":"2023-10-04T12:10:13","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=5325"},"modified":"2026-02-09T14:30:46","modified_gmt":"2026-02-09T14:30:46","slug":"discharge-hub-referral-form","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=5325","title":{"rendered":"Discharge Hub referral form"},"content":{"rendered":"<p><strong>What is it?<\/strong><\/p>\n<ul>\n<li>The Discharge Hub Referral form has been added to eRecord to replace the current word document.<\/li>\n<li>As it is created from a patient record, this will improve data quality of referrals received into the hub.<\/li>\n<li>As it is created in the patient record it will improve the accessibility of the form and the time it takes to complete for ward staff.<\/li>\n<\/ul>\n<p><strong>Access Ward Dashboard:<\/strong><\/p>\n<p>In Powerchart<\/p>\n<ul>\n<li>Click <strong>View<\/strong> then <strong>Ward Dashboard <\/strong>&#8211; you should also be able to see and click on Ward Dashboard in the row below.<\/li>\n<\/ul>\n<p><strong>Access Discharge Dashboard and opening the form:<\/strong><\/p>\n<ol>\n<li>Select\u00a0 the <strong>Discharge <\/strong>tab.<\/li>\n<li>Click on the <strong>Documentation<\/strong> column for the Patient.<\/li>\n<li>Click the <strong>Create<\/strong> button.<\/li>\n<li>Select <strong>Discharge Hub Referral<\/strong> from the drop-down window to open the form.<\/li>\n<li>The form has two pages which have <strong>mandatory fields<\/strong> that need to be completed before you can sign the form, these are highlighted with a red asterisk.<\/li>\n<li>The mandatory fields in each page are highlighted <strong>yellow<\/strong> and will provide the right level of information for the discharge hub team.<\/li>\n<li>The Package of care section is not mandatory and should only be completed by an Occupational Therapist or the IDT team when it applies to the patient.<\/li>\n<li>To sign the form, click the <strong>green tick<\/strong> in the top left-hand corner \u2013 <strong>this will automatically create a task for the hub team to review the details<\/strong>.<\/li>\n<\/ol>\n<p><span data-teams=\"true\"><div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  <\/span>Once the form has been saved it can be accessed from the Discharge Dashboard and added to later.<span data-teams=\"true\"><\/div><\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>How to open, access, and complete the Discharge Hub referral form<\/p>\n","protected":false},"author":4,"featured_media":1924,"parent":5309,"menu_order":0,"comment_status":"closed","ping_status":"closed","template":"qrg.php","meta":{"inline_featured_image":false,"footnotes":""},"categories":[14,15],"tags":[],"class_list":["post-5325","page","type-page","status-publish","has-post-thumbnail","hentry","category-erecord","category-powerchart"],"jetpack_sharing_enabled":true,"_links":{"self":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/5325","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages"}],"about":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/types\/page"}],"author":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/users\/4"}],"replies":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Fcomments&post=5325"}],"version-history":[{"count":16,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/5325\/revisions"}],"predecessor-version":[{"id":20691,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/5325\/revisions\/20691"}],"up":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/5309"}],"wp:featuredmedia":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/media\/1924"}],"wp:attachment":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Fmedia&parent=5325"}],"wp:term":[{"taxonomy":"category","embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Fcategories&post=5325"},{"taxonomy":"post_tag","embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Ftags&post=5325"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
