{"id":5319,"date":"2023-10-04T11:31:27","date_gmt":"2023-10-04T11:31:27","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=5319"},"modified":"2026-02-09T13:13:28","modified_gmt":"2026-02-09T13:13:28","slug":"board-round-form","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=5319","title":{"rendered":"Board Round Form"},"content":{"rendered":"<p><strong>What is it?<\/strong><\/p>\n<ul>\n<li>The Discharge Planning &#8211; board round form should be updated daily by the MDT team on the ward for all adult inpatients.<\/li>\n<li>This allows key dates to be captured along with the reason to reside and the discharge pathway that the patient is on.<\/li>\n<li>There is embedded guidance about discharge pathways in the form.<\/li>\n<\/ul>\n<p><strong>How do I access and open the board round form?<\/strong><\/p>\n<ol>\n<li>In the Patient record, click on <strong>AdHoc<\/strong>.<\/li>\n<li>Select <strong>Discharge Planning &#8211; Board Round.<\/strong><\/li>\n<li>Select <strong>Record<\/strong>.<\/li>\n<\/ol>\n<p><strong>How do I complete the form?<\/strong><\/p>\n<ol>\n<li>Expected Date of Discharge <strong>should <\/strong>be entered for all inpatients that will stay overnight. This is mandatory in the form.<\/li>\n<li>Planned Discharge Time <strong>can be entered if known<\/strong> to assist with discharge planning on the patient\u2019s final day.<\/li>\n<li>Discharge Ready Date is the <strong>first date<\/strong> that the patient did not meet the Criteria to reside.<\/li>\n<li>When a patient meets the criteria to reside in hospital, <strong>one reason must be selected<\/strong>.<\/li>\n<li>If the patient no longer meets the criteria to reside, the <strong>discharge ready date should be entered<\/strong>.<\/li>\n<li>If there is a clinical exception that means the patient would remain in hospital for another night, <strong>this can be recorded in the form<\/strong>.<\/li>\n<li>The discharge pathway should be entered to allow monitoring and performance data to be collected.<\/li>\n<li>The Hub Referral question is a prompt only to consider completing the hub referral form.<\/li>\n<\/ol>\n<p><span data-teams=\"true\"><div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  <\/span>More information about discharge pathways can be found in the Pathway Guidance section of the form.<span data-teams=\"true\"><\/div><\/span><\/p>\n<p><span style=\"color: white;\">.
