{"id":5311,"date":"2023-10-04T09:51:33","date_gmt":"2023-10-04T09:51:33","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=5311"},"modified":"2026-03-18T14:57:56","modified_gmt":"2026-03-18T14:57:56","slug":"discharge-dashboard","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=5311","title":{"rendered":"Ward Dashboard"},"content":{"rendered":"<p>How to access Ward Dashboard:<\/p>\n<p>In Powerchart.<\/p>\n<ul>\n<li>Click <strong>View<\/strong> then <strong>Ward Dashboard &#8211; <\/strong>you should also be able to see and click on <strong>Ward Dashboard <\/strong>in the row below.<\/li>\n<\/ul>\n<p>How to access the Discharge Dashboard:<\/p>\n<ul>\n<li>Click on the\u00a0<strong>Discharge\u00a0<\/strong>tab.<\/li>\n<li>The <strong>Discharge Dashboard<\/strong> is a list that summarises the discharge plans for a group of patients.<\/li>\n<li>It is the place in <strong>eRecord<\/strong> to see and update information about a patient\u2019s discharge.<\/li>\n<li>The list of patients can be sorted by the expected date of discharge to see who is almost ready to be discharged on a ward.<\/li>\n<li>You can complete the new Discharge Hub Referral form from this page.<\/li>\n<\/ul>\n<p><strong>How to set it up:<\/strong><\/p>\n<ol>\n<li>Click\u00a0<strong>Select List<\/strong>.<\/li>\n<li>Click <strong>Patient list<\/strong>.<\/li>\n<li>Click\u00a0<strong>List Maintenance<\/strong>.<\/li>\n<li>In the opening Patient list window, any lists you have already set up for Care Compass will appear in the <strong>Available lists<\/strong> column.<\/li>\n<li>Select the list you want to set up, then click the right arrow in the middle to pull it across into the <strong>Active lists<\/strong> column.<\/li>\n<li>Click <strong>OK<\/strong>.<\/li>\n<li>The list should now be appearing and a list of Patient\u2019s from that group will be showing.<\/li>\n<\/ol>\n<p><strong>How to establish relationships:<\/strong><\/p>\n<ol>\n<li>Your <strong>Patient List<\/strong> will now be showing.<\/li>\n<li>Some patients may have inactive fields (boxes will appear grey).<\/li>\n<li>Click <strong>Establish Relationships<\/strong>.<\/li>\n<li>Click \u2018<strong>select all<\/strong>\u2019 and then \u2018<strong>submit<\/strong>\u2019.<\/li>\n<li>The page will reload, this may take a few moments.<\/li>\n<\/ol>\n<p>The dashboard will give you a ward overview of the discharge status of the patients on your ward. These include:<\/p>\n<ul>\n<li><strong>Patient<\/strong> &#8211; patients name and MRN<\/li>\n<li><strong>Location<\/strong> &#8211; Ward number and bedspace<\/li>\n<li><strong>Visit<\/strong> &#8211; Admission date and length of stay<\/li>\n<li><strong>Isolation<\/strong> &#8211; Isolation status<\/li>\n<li><strong>Actions<\/strong> &#8211; This can be used as a free text box to update outstanding MDT discharge actions. E.g., \u2018Equipment being delivered 25th September\u2019, \u2018Stair assessment needed\u2019, \u2018District nurse referral required\u2019. Information entered here will not pull into the patients record, clinical notes should be recorded as usual.<\/li>\n<li><strong>Pathway<\/strong> &#8211; Discharge pathway<\/li>\n<li><strong>Expected Date of Discharge<\/strong> &#8211; Current expected date that the patient will be able to leave hospital.
