How to discharge a Postnatal Inpatient to Community<\/p>\r\n\r\n\r\n\r\n

  1. Open mother\u2019s record in BadgerNet.<\/li>\r\n\r\n\r\n\r\n
  2. Click on the Postnatal <\/strong>tab.<\/li>\r\n\r\n\r\n\r\n
  3. From the Add new note<\/strong> dropdown, select Transfer of Care Smart Form (Woman)<\/strong>.<\/li>\r\n\r\n\r\n\r\n
  4. In the Transfer of Care Smart Form, select Postnatal Community Care for the Transfer Type<\/strong> field.<\/li>\r\n\r\n\r\n\r\n
  5. Complete details then select Save and Close<\/strong><\/li>\r\n\r\n\r\n\r\n
  6. Under Reports<\/strong>, select Clinical Reports<\/strong>.<\/li>\r\n\r\n\r\n\r\n
  7. Select Admissions and Discharge<\/strong>, then Mother Transfer to Postnatal Community Care<\/strong>.<\/li>\r\n\r\n\r\n\r\n
  8. Click Confirm and Save<\/strong> in the top right corner of the form.<\/li>\r\n\r\n\r\n\r\n
  9. Under \u2018Baby 1<\/strong>\u2019 tab, click on \u2018Summary of Care<\/strong>\u2019.<\/li>\r\n\r\n\r\n\r\n
  10. From the Add new note dropdown<\/strong>, select Transfer of Care Smart Form (Baby)<\/strong>.<\/li>\r\n\r\n\r\n\r\n
  11. Complete details, then select Save and Close<\/strong>.<\/li>\r\n\r\n\r\n\r\n
  12. Under \u2018Baby 1<\/strong>\u2019 tab, click on \u2018Reports<\/strong>\u2019.<\/li>\r\n\r\n\r\n\r\n
  13. Select Admissions\/Transfer, then Baby Transfer to Postnatal Community Care<\/strong>.<\/li>\r\n\r\n\r\n\r\n
