How to discharge an Antenatal Inpatient to Community<\/p>\r\n\r\n\r\n\r\n

DO NOR USE THE TRANSFER OF CARE FORM\/ NOTE FOR ANTENATAL DISCHARGES<\/mark><\/strong><\/p>\r\n\r\n\r\n\r\n

  1. Search for \u2018Medical Discharge\u2019<\/strong> in the \u2018Enter new note\u2019<\/strong> feature.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n

      \r\n
    1. In form, complete summary details of inpatient stay, then authorise and sign form.<\/li>\r\n\r\n\r\n\r\n
    2. Click on \u2018Clinical Reports\u2019<\/strong> in the \u2018reports\u2019 section.<\/li>\r\n\r\n\r\n\r\n
    3. Select \u2018Medical Discharge Letter\u2019 <\/strong>under the \u2018Admission and Discharge\u2019 <\/strong>heading.<\/li>\r\n\r\n\r\n\r\n
    4. Click on Confirm and Save<\/strong> in the top right corner of the form to send this report to the admin in box (email will already be pre-populated nuth.postnataldischarges@nhs.net)<\/li>\r\n\r\n\r\n\r\n
    5. This drops into mailbox monitored by the admin team who will then email this out to the community midwives along with the postnatal discharges<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n
