{"id":4234,"date":"2022-12-05T10:58:07","date_gmt":"2022-12-05T10:58:07","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=4234"},"modified":"2026-05-15T14:38:23","modified_gmt":"2026-05-15T14:38:23","slug":"how-to-print-a-dnacpr-form","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=4234","title":{"rendered":"How to create and print a DNACPR form"},"content":{"rendered":"\r\n<p class=\"wp-block-paragraph\">Follow these steps to create and print a new DNACPR form:<\/p>\r\n<ol>\r\n<li>Open the patient record and click <strong>Document Creation<\/strong> and in the main menu<\/li>\r\n<li>In the Encounter box ensure the <strong>correct encounter<\/strong> for this visit is selected<\/li>\r\n<li>In the Letter Type box select <strong>DNACPR-Form<\/strong><\/li>\r\n<li>Click <strong>Create<\/strong><\/li>\r\n<li>The details recorded in the DNACPR form will <strong>automatically<\/strong> populate from the patient&#8217;s Treatment Escalation Plan.\u00a0 C<strong>heck this is<\/strong><br \/><strong>correct and authorised by a consultant.<\/strong><\/li>\r\n<li>On the <strong>Details tab<\/strong>\u00a0set the <strong>Lead Clinician<\/strong> and <strong>Signing Clinician<\/strong> to the authorising consultant:<\/li>\r\n<li>Click <strong>Save and Finalise:<\/strong><\/li>\r\n<li>The screen will refresh and a green bar will display at the bottom. Select <strong>click here\u00a0<\/strong>to print a copy:<\/li>\r\n<li>The document will open in a new window. Click <strong>Printer<\/strong> icon.<\/li>\r\n<li>The \u2018Print\u2019 screen will open.
