{"id":4132,"date":"2022-12-05T07:29:47","date_gmt":"2022-12-05T07:29:47","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=4132"},"modified":"2025-11-21T18:10:33","modified_gmt":"2025-11-21T18:10:33","slug":"dnacpr-overview","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=4132","title":{"rendered":"DNACPR &#8211; Overview"},"content":{"rendered":"\r\n<p>&nbsp;<\/p>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\">\r\n<li>Open a <strong>Patient&#8217;s<\/strong> record within <strong>PowerChart<\/strong>, click <strong>Treatment Escalation Plan<\/strong> in the blue menu.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"2\">\r\n<li>This will display the details of the DNACPR status and show when the appropriate documents were created and by whom.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"3\">\r\n<li>Click on the plus button and select a document to <strong>Add <\/strong>or <strong>Modify<\/strong> from the list.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"4\">\r\n<li>Some of the forms available are:<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li><strong>Treatment Escalation Plan and Resuscitation Status<\/strong> &#8211; Use this form to document a Treatment Escalation Plan and\/or a DNCAPR decision.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li><strong>DNACPR Review<\/strong> &#8211; Use this form to review a previous DNACPR decision, you can confirm that DNACPR is still valid or that it needs to be rescinded.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"5\">\r\n<li>To counter sign the <strong>TEP<\/strong> form &#8211; Under <strong>Details<\/strong> select the <strong>TEP<\/strong> form and select <strong>Modify<\/strong> on the right hand side.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"6\">\r\n<li>Select<strong> Counter Sign Now <\/strong>mandatory yellow fields which will enable you to record your details.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"7\">\r\n<li>The form will update the blue banner bar, at the top of the patient&#8217;s record and will show the current DNACPR status, date and time.<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<p>&nbsp;<\/p>\r\n\r\n\r\n\r\n<p>&nbsp;<\/p>\r\n","protected":false},"excerpt":{"rendered":"<p>How to complete the DNACPR process.<\/p>\n","protected":false},"author":4,"featured_media":1657,"parent":2231,"menu_order":0,"comment_status":"closed","ping_status":"closed","template":"qrg.php","meta":{"inline_featured_image":false,"footnotes":""},"categories":[14,15],"tags":[],"class_list":["post-4132","page","type-page","status-publish","has-post-thumbnail","hentry","category-erecord","category-powerchart"],"jetpack_sharing_enabled":true,"_links":{"self":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/4132","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages"}],"about":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/types\/page"}],"author":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/users\/4"}],"replies":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Fcomments&post=4132"}],"version-history":[{"count":17,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/4132\/revisions"}],"predecessor-version":[{"id":21162,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/4132\/revisions\/21162"}],"up":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/pages\/2231"}],"wp:featuredmedia":[{"embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=\/wp\/v2\/media\/1657"}],"wp:attachment":[{"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Fmedia&parent=4132"}],"wp:term":[{"taxonomy":"category","embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Fcategories&post=4132"},{"taxonomy":"post_tag","embeddable":true,"href":"https:\/\/training.nuth-itt.co.uk\/index.php?rest_route=%2Fwp%2Fv2%2Ftags&post=4132"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
