Overview of DNACPR documentation in the patient record.<\/p>\n

  1. With a Patient’s<\/strong> record open, click Treatment Escalation Plan<\/strong> in the blue menu.<\/li>\n
  2. This will display the Advance Care Planning area which displays the details of the DNACPR status<\/strong> and show when the related documents were created and by whom.<\/li>\n
  3. Click on the plus button and select a document to Add <\/strong>or Modify<\/strong> from the list.<\/li>\n
  4. Some of the forms available are:\n

    • Treatment Escalation Plan and Resuscitation Status<\/strong> – Use this form to document a Treatment Escalation Plan and\/or a DNCAPR decision.<\/li>\n
    • Annual review of existing DNACPR <\/strong>\u00a0– Use this form to review a previous DNACPR decision, you can confirm that DNACPR is still valid or that it needs to be rescinded.<\/li>\n<\/ul>\n<\/li>\n
    • To counter sign the TEP<\/strong> form – Under Details<\/strong> select the TEP<\/strong> form and select Modify<\/strong> on the right hand side.<\/li>\n
    • Select Counter Sign Now <\/strong>mandatory yellow fields which will enable you to record your details. The form will update the blue banner bar, at the top of the patient’s record and will show the current DNACPR status, date and time.<\/li>\n<\/ol>\n","protected":false},"excerpt":{"rendered":"
