<\/span>Accessing the Antenatal Booking Form<\/span><\/h3>\r\n\r\n\r\n\r\n

  1. In the pregnancy record, select Notes During Pregnancy from the left menu<\/li>\r\n
  2. Click the red plus button to the top left \"\"<\/li>\r\n
  3. From the drop-down menu select Antenatal Booking<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n

    <\/span>Overview of the Antenatal Booking Form<\/span><\/h3>\r\n\r\n\r\n\r\n

    The Antenatal Booking form is a smart form that allows you to complete several separate forms at the same time. The sections to the left of the form can be worked through one after the other – there is no need to save and close the form after completing each section<\/span>!<\/p>\r\n\r\n\r\n\r\n

    1. On Arrival<\/strong> – start date and time of the booking, location and type of assessment. Also allows import of information from the Portal Pre-Booking form<\/li>\r\n
    2. About the Woman<\/strong> – access to the Portal Access and Demographics forms<\/li>\r\n
    3. Current Pregnancy Details<\/strong> – dates of LMP, Agreed EDD, Referral and Menstrual, Fertility and Contraception information<\/li>\r\n
    4. Obstetric History<\/strong> – allows import of previously recorded Obstetric History if this is available on BadgerNet<\/li>\r\n
    5. Assessment<\/strong> – record Observations<\/li>\r\n
    6. Health History <\/strong>– Medical History including Allergies, Mental Health, Family Health, Smoking and Alcohol<\/li>\r\n
    7. Social and Confidential<\/strong> – Safeguarding information including FGM.<\/li>\r\n
    8. Screening<\/strong> – Booking Bloods & Microbiology screens, Vaccinations and Scans<\/li>\r\n
    9. VTE Risk Assessment<\/strong> – Record the VTE risk<\/li>\r\n
    10. Recommendations<\/strong> – System generated Risk Assessment & Management Plan that may be modified by the user<\/li>\r\n
    11. Fetal Growth and Pre-eclampsia Risk Assessment<\/strong><\/li>\r\n
    12. Care Plan <\/strong>– Named Midwife and other professionals involved in care, intended location of birth<\/li>\r\n
    13. Discussion<\/strong> – Record information discussed with the woman<\/li>\r\n
    14. On Leaving<\/strong> – Assessment end date and time. If the referral needed checkbox is set to yes a Referral Form will open automatically when the Antenatal Booking form is Saved and Closed<\/li>\r\n<\/ol>\r\n<\/div>\r\n<\/div>\r\n","protected":false},"excerpt":{"rendered":"
