The dashboard will give you a ward overview of the discharge status of the patients on your ward.

  1. Patient<\/strong> – patients name and MRN <\/li>\n
  2. Location<\/strong> – Ward number and bedspace <\/li>\n
  3. Visit<\/strong> – Admission date and length of stay <\/li>\n
  4. Isolation<\/strong> – Isolation status<\/li>\n
  5. Actions<\/strong> – This can be used as a free text box to update outstanding MDT discharge actions. E.g., \u2018Equipment being delivered 25th September\u2019, \u2018Stair assessment needed\u2019, \u2018District nurse referral required\u2019. Information entered here will not pull into the patients record, clinical notes should be recorded as usual. <\/li>\n
  6. Pathway<\/strong> – Discharge pathway<\/li>\n
  7. Expected Date of Discharge<\/strong> – Current expected date that the patient will be able to leave hospital. Use this field to update the board round form.<\/li>\n<\/li>\n
  8. Ready for Discharge<\/strong> – Date the person no longer meets the criteria to reside. <\/li>\n
  9. Resus<\/strong> – Resuscitation status.<\/li>\n
  10. Discharge medications<\/strong> – Progress of discharge medications within pharmacy. <\/li>\n
  11. Notes<\/strong>\t<\/li>\n
  12. Documentation<\/strong> – Indicates if a board round form or discharge hub referral form has been completed.<\/li>\n<\/ol>\n

    Discharge Tab Utility<\/div>
    \n\u2022 The Discharge Tab allows you to set up a list that summarises the discharge plans for a group of patients.
    \n\u2022 It is the place in eRecord to see and update information about a patient\u2019s discharge.
    \n\u2022 The list of patients can be sorted by the expected date of discharge to see who is almost ready to be discharged on a ward.
    \n\u2022 You can complete the new Discharge Hub Referral form from this page.
