{"id":17398,"date":"2025-10-05T11:50:25","date_gmt":"2025-10-05T11:50:25","guid":{"rendered":"https:\/\/training.nuth-itt.co.uk\/?page_id=17398"},"modified":"2026-02-09T12:48:35","modified_gmt":"2026-02-09T12:48:35","slug":"how-to-add-problems-to-a-patient-record-2","status":"publish","type":"page","link":"https:\/\/training.nuth-itt.co.uk\/?page_id=17398","title":{"rendered":"How to Add Problems to a Patient Record"},"content":{"rendered":"<p>You are within a patient&#8217;s record in Powerchart:<\/p>\n<ol>\n<li>Click on <strong>Flags<\/strong> in banner bar at top of record or <strong>Diagnosis &amp; Problems<\/strong> in the blue menu on the left side of the record.<\/li>\n<li>Click <strong>+Add<\/strong> in the Problems section in the lower half of the window.<\/li>\n<li>Locate problem in <strong>Folders<\/strong> and double click on it.<\/li>\n<li>Complete the following fields:\n<ul>\n<li>Responsible Clinical Staff<\/li>\n<li>Onset: Date<\/li>\n<li>Confirmation<\/li>\n<li>Classification<\/li>\n<li>Status<\/li>\n<\/ul>\n<\/li>\n<li>To save problem to patient&#8217;s record, click:\n<ul>\n<li><strong>OK<\/strong> to add just this problem and close the window<\/li>\n<li><strong>OK &amp; Add New<\/strong> to add this problem and open the window to allow entry of another problem<\/li>\n<li><strong>Add Problem &amp; Diagnosis<\/strong> to add this problem to both, Problem and Diagnosis, sections<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><span data-teams=\"true\"><div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  <\/span>Problems with a classification of Clinical Risk, Safeguarding, and Accessibility will appear on the banner bar.
