All &#8216;Active&#8217; Short Patient\/Carecompass lists will display.<\/li>\n<p><img loading=\"lazy\" decoding=\"async\" fields that need to be completed before you can sign the form, these are highlighted with a <strong>red asterisk<\/strong>.<\/li>\n<li>The mandatory fields in each page are highlighted <strong>yellow<\/strong> and will provide the right level of information for the discharge hub team. <\/li>\n<p><img loading=\"lazy\" decoding=\"async\" <strong>green tick<\/strong> in the top left-hand corner \u2013 <strong><em>this will automatically create a task for the hub team to review the details<\/em><\/strong>.<br \/>\n<img loading=\"lazy\"
