Creating an Encounter: Note<\/h1>\n<div class=\"guideSection\" id=\"\">Background<\/div>\n<p>It can be necessary to record information in the patient record even when a formal clinical encounter has not occurred.\u00a0 This is usually best done in &#8220;Note&#8221;.<\/p>\n<div class=\"guideSection\" id=\"\">Process<\/div>\n<ul>\n<li><a href=\"https:\/\/training.nuth-itt.co.uk\/?page_id=14580\">Open mediSIGHT and find the patient record<\/a><\/li>\n<li>Click &#8220;Create new +&#8221;
