Background<\/div>\n

When a patient does not arrive for their appointment (clinic, theatre or any other appointment) it is important to make a record in the notes.\u00a0 This can assist in enquiries later, or highlight when a patient is in need of additional support. This may include WNB “Was Not Brought”, DNA “Did Not Attend”, UTA “Unable To Attend”.<\/p>\n

It may be desirable to also generate a letter to the patient.\u00a0 Currently this should be done in DocStore in Powerchart.<\/p>\n

Please refer to the NUTH WNB Policy<\/a> which can be found on InPhase<\/a>, and the NUTH Patient Access Policy on InPhase<\/a><\/p>\n

<\/i> Note that this guide refers to documenting the DNA, and clinical decision regarding review.\u00a0 The documentation on mediSIGHT does not have any effect on patient care.\u00a0 It is critical that an 18 week wait tracker form is completed, and any correspondence is created in DocStore.<\/div>\n

  1. Open mediSIGHT and find the patient.<\/a><\/li>\n
  2. Click “Create new +” and choose “DNA”<\/li>\n<\/ol>\n

    1. Ensure the correct\u00a0Episode<\/strong> and\u00a0Pathway<\/strong> are chosen.<\/li>\n
    2. Choose and appropriate\u00a0Reason<\/strong><\/li>\n
    3. Click “Plan” and add a note stating the DNA and whether the patient is to be discharged or rebooked.<\/li>\n
    4. If the patient is to be discharged, ensure that you click\u00a0Discharge<\/strong> to close the pathway<\/li>\n<\/ol>\n

      1. Click “Finalise” to save the record.<\/li>\n<\/ol>\n
        Writing to the patient \/ GP<\/div>\n

        1. Letters to the patient \/ GP are not currently generated by mediSIGHT<\/li>\n
