This guide shows how to complete the self-referral form for the occupational health service.<\/p>\n

To access the self-referral portal please visit the Occupational Health<\/strong> page on the intranet and select Self-referral information<\/strong> in the menu.<\/p>\n

  1. A message will display. Please read the message<\/strong> as it contains important information related to your referral. When you are ready click Continue<\/strong>.<\/li>\n<\/ol>\n

    1. Complete your personal details<\/strong> including name, date of birth and telephone.<\/li>\n<\/ol>\n

      1. Select the reason<\/strong> for your request in the drop-down menu. The options are counselling or physiotherapy.<\/li>\n<\/ol>\n

        1. The referral information section<\/strong> for the selected type of referral will display.<\/li>\n
        2. Read the information provided and complete the details<\/strong>.<\/li>\n
        3. Each referral form has questions specific to the type of referral being requested. Mandatory questions are marked with a red star.<\/li>\n
        4. At the bottom you can attach any related documents<\/strong>. Drag and drop the file or use the Browser for files button. Once attached the file will display below. It is not mandatory to attach documents.<\/li>\n<\/ol>\n

            1. A confirmation message<\/strong> will display. The occupational health team will be in touch once the request has been reviewed.<\/li>\n<\/ol>\n
