VTE Risk Assessment<\/h1>\n
Scenario<\/div>\n

Trust policy requires all patients 16 years or older to have a VTE Risk Assessment completed on admission and when clinical condition changes during the admission.<\/p>\n

Completing the assessment via AdHoc Forms<\/div>\n
    \n
  1. Click on the\u00a0AdHoc<\/strong>\u00a0button on the menu bar at the top of the screen\u00a0\u00a0\"AdHoc.<\/li>\n
  2. Select\u00a0VTE Assessment NUTH.<\/strong><\/li>\n
  3. Click the\u00a0Record\u00a0<\/strong>button.<\/li>\n
  4. Complete<\/strong>\u00a0the assessment and\u00a0sign.<\/strong><\/li>\n<\/ol>\n

    Alert after 24 hours <\/div>\n

    If the VTE risk assessment has not been completed within 24 hours<\/strong> of admission an alert will be displayed when the patient record is opened.<\/p>\n

      \n
    1. User opens patient record.<\/li>\n
    2. Completion of VTE Risk Assessment required warning box<\/strong> displays.\n
        \n
      • Click the VTE Risk Assessment button<\/strong> to complete the form.<\/li>\n
      • To cancel opening the record select Cancel and close record\u00a0<\/strong>and click OK.<\/strong><\/li>\n
      • To proceed without completing the form select Emergency Override<\/strong> and click OK.<\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ol>\n

        Alert when prescribing <\/div>\n

        The VTE Risk Assessment must be documented in the record before anticoagulant medication can be prescribed. If it has not been documented the user will be prompted to do so before continuing.<\/p>\n

          \n
        1. User attempts to prescribe anticoagulant medication.<\/li>\n
        2. Completion of VTE Risk Assessment required warning box<\/strong> displays.\n
            \n
          • Click the VTE Risk Assessment button<\/strong> to complete the form.<\/li>\n
          • To cancel prescribing the medication tick the CANCEL ORDER<\/strong> box and click OK.<\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ol>\n

            <\/i> If the warning has been cancelled or overridden it will not display again for 2 hours. The assessment can be completed at any time via AdHoc forms.<\/div>\n
