All yellow fields are Mandatory fields. You must complete these fields in order to add the admission (if you are not registering a new patient, check the demographic details are correct and update if required using the steps below).<\/li>\r\n<li>If you cannot see the mandatory fields, right-click anywhere in the grey area of the form and select <strong>Highlight Required Fields<\/strong> <img loading=\"lazy\" decoding=\"async\" width=\"150\" height=\"39\" class=\"wp-image-3126\" style=\"width: 150px;\" src=\"https:\/\/training.nuth-itt.co.uk\/wp-content\/uploads\/2022\/04\/PMOffice-Highlight-Required-Fields.png\" alt=\"\" \/><br \/><br \/><\/li>\r\n<li>Add the Patient <strong>NHS Number <\/strong>\u2013 although this is not a mandatory field, if the patient NHS Number is known, it should be added to the patient record. If you had searched for your patient using initials and Date of Birth, you must complete the patient <strong>Surname <\/strong>and <strong>First Name<br \/><br \/><\/strong><\/li>\r\n<li>Complete the following fields:\r\n<ol>\r\n<li><strong>Date of Birth<\/strong><\/li>\r\n<li><strong>Gender<\/strong><\/li>\r\n<li><strong>Ethnic Category<\/strong><\/li>\r\n<li><strong>UK Resident for 12 months?<\/strong><\/li>\r\n<li><strong>Administrative Category<\/strong> (this will default to NHS if you selected Yes to the previous &#8216;UK Resident for 12 months?&#8217; question)<\/li>\r\n<\/ol>\r\n<\/li>\r\n<\/ol>\r\n<h3><span class=\"ez-toc-section\" id=\"GP_Details\"><\/span><div class=\"guideSection\" id=\"\">GP Details<\/div><span class=\"ez-toc-section-end\"><\/span><\/h3>\r\n<!-- \/wp:post-content -->\r\n\r\n<!-- wp:list {\"ordered\":true,\"start\":1} -->\r\n<ol start=\"1\">\r\n<li>The next field to complete is the GP. Select either &#8220;GP practice not known&#8221; or &#8220;patient does not have a GP&#8221; from the search results, and press OK.<\/div>\r\n<!-- \/wp:list -->\r\n\r\n<!-- wp:heading {\"level\":3} -->\r\n<div class=\"guideSection\" id=\"\">Telephone Number<\/div>\r\n<!-- \/wp:heading -->\r\n\r\n<!-- wp:list {\"ordered\":true,\"start\":1} -->\r\n<ol start=\"1\">\r\n<li>The options are Home Phone \/ Mobile phone \/ Work Phone<strong>. If the patient phone number is not known, or they do not have a telephone, YOU MUST ENTER NK<\/strong> into the home phone field. Close this by clicking <strong>OK <\/strong>and return to the Add Patient screen<\/li>\r\n<\/ol>\r\n<div class=\"infoSection\"><i class=\"fa fa-info-circle\" style=\font-size:3rem\"><\/i>  Adding Overseas addresses and other scenarios<em>\u00a0can be found in our support guide <a href=\"https:\/\/training.nuth-itt.co.uk\/?page_id=5031\">Add or Update Patient Demographics<\/a><\/em><\/div>\r\n<p><strong>Once the demographics have been added, the visit details will need to be completed.<\/strong> \u00a0<\/p>\r\n<p><strong><div class=\"guideSection\" id=\"\">Visit Details Tab<\/div>\u00a0<\/strong><\/p>\r\n<p>From the <strong>Visit Details Tab<\/strong>,\u00a0 <strong><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-13367\" src=\"https:\/\/training.nuth-itt.co.uk\/wp-content\/uploads\/2025\/04\/Visit-Details-tab.png\" alt=\"\" width=\"216\" height=\"35\" \/> <\/strong>complete the missing fields:<\/p>\r\n<ol>\r\n<li><em>Admission Method: <\/em>Who sent the patient to your ward<\/li>\r\n<li><em>Source of Admission:\u00a0<\/em>Where the patient has come from (if it&#8217;s their home address; select Usual Place of Residence)<\/li>\r\n<li><em>Referring Clinician: \u00a0<\/em>Use the magnifying glass to locate the referring clinician typing the surname and first name of the referring clinician.\u00a0 If the referring clinician is not found, use <strong>NotKnown \u2013 Referring Clinician Not Known<\/strong><\/li>\r\n<li><em>Lead Clinician:<\/em> Type the<strong> Surname of the Lead Clinician<\/strong>, click the <strong>magnifying glass, double click <\/strong>to select the correct clinician<\/li>\r\n<li><em>Treatment Function:<\/em> Select the <strong>correct treatment<\/strong> from the dropdown list<\/li>\r\n<li><em>Ward:\u00a0<\/em>Click the dropdown and select the ward the patient is being admitted to &#8211; click the\u00a0<strong>Bedboard tab<\/strong><\/li>\r\n<li>Select the Bay \/Bed for the patient, click <strong>OK<\/strong><\/li>\r\n<li><em>Visit Date:<\/em><strong>\u00a0<\/strong>Enter a &#8220;<strong>T<\/strong>&#8221; to add today&#8217;s date<\/li>\r\n<li><em>Visit Time:<\/em> Enter a &#8220;<strong>N<\/strong>&#8221; to add the current time (now)<\/li>\r\n<\/ol>\r\n<p><strong><div class=\"guideSection\" id=\"\">Next of Kin Tab<\/div>\u00a0<\/strong><\/p>\r\n<p>From the Next of Kin Tab, <strong><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-13368\" src=\"https:\/\/training.nuth-itt.co.uk\/wp-content\/uploads\/2025\/04\/Next-of-Kin-tab.png\" alt=\"\" width=\"283\" height=\"36\" \/>\u00a0<\/strong> search for the next of kin:<\/p>\r\n<ol>\r\n<li>Complete the NOKCONFIRMATION field.\u00a0 If <em>Patient Refused to Provide NOK Confirmation<\/em> or\u00a0<em>Unable to Provide NOK<\/em> is selected, click\u00a0<strong>OK\u00a0<\/strong>to complete the admission.\u00a0 For\u00a0<em>NOK Confirmed<\/em> you will need to complete further information (see below)<\/li>\r\n<li><strong>NOK Confirmed<\/strong><br \/>\r\n<ol>\r\n<li>Complete the\u00a0<strong>NOKs Relationship to Patient<\/strong> field<\/li>\r\n<li>Click the\u00a0<strong>Search for Next of Kin\u00a0<\/strong>button to search for the next of kin.\u00a0 If the next of kin is known to the Trust, select the person and click <strong>OK<\/strong> (if the next of kin is a known patient, their demographic details will populate and the admission can be completed by selecting <strong>OK<\/strong> at the bottom right of the screen).
