<\/span>Standard Operating Procedure<\/span><\/h2>\n

<\/span>Purpose: To provide clear instructions to clinical staff in the documentation and security of patient keycodes.<\/span><\/h2>\n<\/th>\n<\/tr>\n

1<\/strong><\/td>\nExplain Rationale and Purpose<\/strong><\/td>\n\n
2<\/strong><\/td>\nKeycode information shared to District Nursing Service<\/strong><\/td>\n\n

  • Shared sources could include, Social Care Direct, NuTH Services, patients, family\/carers. In line with patients consent to share details.<\/li>\n
  • Receiving professionals include Registered Nurses, Nurse Associates and Health Care Assistants.<\/li>\n
  • If the key code will be required for visiting professionals to gain access move to step 3<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n
3<\/strong><\/td>\nDocumentation of keycode<\/strong><\/td>\n\n
    \n
  • Professional who has been informed of the keycode number should document this in the patients record on SystmOne when the information is shared.<\/li>\n
  • The documentation may take place during the patient visit or once the visit is complete.<\/li>\n
  • Retrieve the patient record and document within nursing assessment template under \u2018maintain safe environment\u2019.<\/li>\n
  • Within the template \u2018patient door access key code\u2019 should be ticked to confirm we have this information<\/li>\n
  • Create a reminder via the link with the template.<\/li>\n
  • Keycode number must be set as a normal priority reminder<\/strong>.<\/li>\n
  • Save consultation.<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n
4<\/strong><\/td>\nInformation for patients<\/strong><\/td>\n\n
    \n
  • Inform the patient verbally this information will be kept securely on their record in line with the Information Security Policy and will be removed when the patient is discharged from the service.<\/li>\n
  • If the patient has capacity issues following a MCA assessment the patients next of kin\/family member should be informed<\/li>\n
  • If this is not possible a best interest\u2019s decision should be made to store the key code to ensure visiting professionals have access and the patient receives the appropriate care.<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n
5<\/strong><\/td>\nKeycode removal<\/strong><\/td>\n\n

  • When the patient is discharged from the service and the referral is ended remove the keycode reminder as part of the discharge process. This can be removed by clicking cancel on the home screen.<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n
